Online forms, documents and e-signatures for patient intake, referrals and consent workflows. The healthcare plan sets up a dedicated account with data encryption, field and folder security, user-level permissions and audit logging; the vendor's help center says this security setup limits connected apps to an approved set, such as Google Drive, Box, Dropbox, Salesforce and Stripe. Formstack is a general form builder; the company now operates as Intellistack.
