Headway's $9.99 marketplace fee: what it costs you, and your 3 options

A flat per-session fee on clients Headway sends you, for as long as you see them. Here is the math, and three calm ways to respond.

Many platform costs are one-time or promotional. This one isn't. Headway's new marketplace service fee comes out of every session with a qualifying client, for the life of that client's care.

If you're on Headway, the question isn't whether the fee is fair. It's what it does to your take-home, and what you want to do about it. Below: what the fee covers as of our research on October 30, 2026, what it costs at a few caseload sizes, and three options with concrete next steps.

Before you act: Headway has already changed these terms more than once. Re-read Headway's help article on the marketplace service fee on the day you make a decision.

What the fee is (and what it isn't)

As of October 30, 2026, based on Headway's help article (checked that day):

  • Amount: $9.99 per session, the same for every health plan. It's deducted from your payout, not billed separately.
  • Who it applies to: sessions with "marketplace" clients — new clients Headway connects you with.
  • How long: for as long as you keep seeing that client, not just the first session.
  • Payment type: both insurance and private-pay sessions.
  • Rollout: state by state starting November 2026, reaching every state by the end of 2026. Headway says it will notify clinicians at least 7 days before the fee starts in their primary practice state.
  • Turning off the marketplace doesn't erase it. Switching off marketplace appearance stops new marketplace clients from finding you. Clients who already booked through the marketplace after your start date stay marketplace clients, and their sessions still carry the fee.

Two changes worth knowing about

The start date moved. Per therapistrates.org, the help article originally listed an October 2026 start for some states. Headway edited it on October 8 to the current November-onward, state-by-state rollout.

The definition widened. Therapistrates.org reports that around October 16, 2026, Headway revised which clients count. The earlier wording covered clients who searched Headway and booked from your profile. The revised wording — now on Headway's help page — covers new clients Headway connects you with through several channels, including marketplace matching, Headway ads, its booking team, connected partners (including health plans and health systems), and AI integrations. A health-system exemption was dropped in the same revision.

What the fee isn't

Per Headway's help article, these sessions are fee-free:

  • Sessions before the fee starts in your state (it isn't applied retroactively)
  • Clients you add yourself through Add Client or the calendar
  • Clients who book through your direct Headway link, wherever you've shared it (your website, directory profile, email signature)
  • Clients who reach your profile directly from a non-ad search engine result or by searching your name on Headway
  • Clients you were already seeing before the fee started in your primary practice state
  • Cancellations and no-shows

Headway also describes the fee as separate from your contracted rate. Practically, it still lowers what you take home per session by $9.99.

These details may have changed since October 30, 2026. Re-check the primary Headway help page before relying on any of them.

What it costs: the math

Because the fee is flat, it takes a bigger bite out of shorter, lower-paying sessions.

Using community-reported median Headway payouts from therapistrates.org:

SessionReported median payout$9.99 fee as % of payout
90837 (60 min)~$119~8.4%
90834 (45 min)~$80~12.5%

Illustrative only. These are unverified community medians across many states and payers, collected before the fee. Your contracted rate may be higher or lower.

Now scale it by how many marketplace-client sessions you see:

Marketplace-client sessions / weekFee / weekFee / month (~4.3 weeks)
5~$50~$215
10~$100~$430
20~$200~$860

Illustrative. Assumes every listed session is fee-bearing.

One weekly example: 10 marketplace sessions at a $119 payout is $1,190 before the fee and about $1,090 after.

One monthly example: at 20 marketplace sessions a week, the fee is roughly $860 a month, or about $10,300 a year if volume holds.

If Headway marketplace clients are most of your panel, this is a lasting cut to take-home, not a temporary promo fee.

Why clinicians are reacting

The reaction has been loud, and it's mostly about structure rather than the dollar amount.

On X, one post objected to a "$10 fee for EVERY patient visit" (Rachael Nomburg). A reply framed platforms as boxing clinicians in (Eliman Dambell) once their caseload depends on them. A Change.org petition asks Headway to pause the rollout and replace the recurring charge with a one-time client-acquisition fee (reported at roughly 1,750 verified signatures as of October 20, 2026; see Medtransic's explainer).

Broader PMC trust context, not specific to this fee: a spring 2025 PsiAN survey found many clinicians weren't told fee-split terms and said they wouldn't use a PMC owned by an insurer. Verify ownership claims independently.

Your three options

None of these is the "right" answer. The best fit depends on how much of your caseload came from Headway's marketplace versus your own referrals.

Option A: Stay, and model the fee into your take-home

Who it fits: clinicians for whom Headway's referral volume, credentialing, and billing still save more time and money than $9.99 a session costs. Also anyone whose caseload is mostly direct-link or self-added clients, since those sessions are fee-free.

First three steps:

  1. After the fee starts in your state (when Headway shows client source), check each client's page for how they found you, so you know which sessions will carry the fee.
  2. Share your direct Headway booking link on your website and directory profiles, since those bookings are exempt.
  3. Re-run your per-session economics with the fee included, using the AIOP comp-split calculator.

Check before deciding: the updated provider agreement Headway says it will share in full, and the 7-day notice for your state.

Option B: Diversify, and don't put 100% of volume on one platform

Who it fits: clinicians who like the PMC model but are uneasy depending on one company whose terms can change mid-year.

The fee is one example of a broader pattern: platform payouts can move under you. Over the summer, Aetna cut pay rates for Alma clinicians effective August 15, 2026 (Clear Health Costs; Matthew Ryan, LCSW). Different platform, different mechanism, same lesson: concentration is a risk.

First three steps:

  1. Calculate what share of your monthly revenue runs through each platform.
  2. Compare the platforms side by side on fees, rates, and terms with the AIOP Headway vs. Alma comparison.
  3. Decide on a ceiling (for example, no single platform above a set share of revenue) and route new referrals accordingly.

Check before deciding: each platform's current rates for your payers and state, and each platform's current help-center terms. Both have changed in 2026.

Option C: Exit, or go more independent

Who it fits: clinicians with enough direct referrals, or the appetite to credential directly with payers, to rely less on any PMC.

Be realistic about friction. Leaving isn't instant: according to Therapy is a Business, clients who came through Headway's marketplace are often expected to stay with Headway, and deactivation can take 30–60 days. Credentialing directly with insurers also takes time.

First three steps:

  1. Separate your caseload into marketplace clients and clients you brought yourself.
  2. Map your timeline: payer credentialing, billing setup, and the deactivation window.
  3. If you're a PMHNP or solo prescriber, work through the AIOP guide to leaving Headway.

Check before deciding: your Headway provider agreement's terms on departing and on marketplace clients, and how long direct credentialing takes with your main payers. Consider a lawyer or accountant for contract and tax questions.

The bottom line

Pick the option that matches your actual mix of marketplace and direct clients, not the loudest post in your feed. A clinician with two marketplace clients and a clinician with twenty are facing very different decisions. Run your own numbers, check your state's start date, and re-read Headway's help article the day you decide, because the terms have already changed twice.
